Codex agents hand over answers that change no file

A Codex agent whose task only reads a file (a row count, say) sometimes named that file in its handover. Obeya refused the handover because the file was not among the card's changes, but Codex never saw the refusal and the answer never arrived. Now Obeya leaves such a file out and still delivers the answer. When an agent's turn ends without a handover, the reminder also repeats any refusal the agent missed.
